With the introduction of nursing care insurance in Germany in the mid-1990s, Avedis Donabedian’s structure-process-outcome model also found its way into nursing homes as quality evaluation tool. However, up to now, there has been no scientifically sound adaptation of this model to the conditions of the German care system.

This book investigates in how far Donadebian’s structure-process-outcome model in its current form is suited to adequately represent the evaluation of quality within German nursing homes. Furthermore, it will answer the question what a quality model suited for inpatient geriatric care should look like instead. A qualitative study as well as an extensive literature review form the basis for the results presented

We will show that Donadebian’s model is hardly suitable as quality model for inpatient geriatric care as it lacks important elements such as social and psychosocial interactions between the actors and stakeholders in nursing homes, as well as reference to the legal and macro-structural parameters. Neither are the alternative models also discussed here suitable for immediate use within the nursing home context.

In order to close this gap, this book identifies those elements that are relevant for the modelling of a new quality model. These have been included in a draft model based on DIN EN ISO 9001:2015, which is a suitable starting point for re-designing, evaluating, and implementing a renewed quality-related model for nursing homes. This can be the basis for replacing the structure-process-outcome model and contribute to the paradigm shift this would necessitate. The discussion of quality in nursing homes can thus take a new and broadened direction and inspire the inter-disciplinary discourse.
